红Help with Medicare costs: MSP, EPIC, Extra Help
Seniors on Medicare with modest income can get three kinds of help: the Medicare Savings Program (MSP) pays monthly premiums, EPIC is New York's senior drug program, and Extra Help lowers prescription costs.

What changed recently
- Under the new federal law, refugees, asylees, TPS holders and some others will lose Medicare (green card holders past 5 years are not affected); Social Security checks don't change. The timeline isn't final; if you get a letter from SSA, see a free lawyer soon.
Who can apply
- MSP: monthly income up to about $2,474 (1) or $3,355 (2) in 2026; New York has no asset test
- EPIC: 65+ with a Part D plan; income up to $75,000 single or $100,000 married
- Extra Help: income up to about $2,015 (single) or $2,725 (couple) a month; assets up to $18,090 or $36,100
How to apply, step by step
- Step 1MSP (Medicare Savings Program) – get the form: use New York form DOH-4328 (Chinese instructions are available). In NYC, call the HRA Helpline at 888-692-6116 for a form, or pick one up at an HRA Medicaid Office, such as the Chinatown office at 115 Chrystie Street, 5th Floor.
- Step 2MSP – attach copies: front and back of your Medicare card, proof of birth date (such as a passport or green card), proof of address (lease, utility bill or postmarked envelope from the last 6 months, not a P.O. Box), proof of income (such as a Social Security or pension letter), and immigration papers if you're not a citizen. If your spouse applies too, include theirs. New York's MSP has no asset test, so no bank statements are needed.
- Step 3MSP – hand it in: in NYC, mail or bring it to HRA; outside NYC, to your county Department of Social Services (DSS). Make a copy of the whole application before you send it.
- Step 4Extra Help (drug cost help) – check if you already have it: people with full Medicaid, MSP or SSI get Extra Help automatically and don't need to apply.
- Step 5Extra Help – apply: apply online at ssa.gov/extrahelp, or call Social Security at 1-800-772-1213 (Mon–Fri 8am–7pm) and ask for an appointment to apply for Extra Help. When you call, stay silent during the English recording until a person answers, then say "Chinese, Mandarin please"; Social Security will get a free interpreter.
- Step 6Extra Help – have ready: bank statements, tax returns, and pension, IRA/401(k) and annuity statements for you and your spouse.
- Step 7EPIC (New York's senior drug program) – get the form: call the EPIC Helpline at 1-800-332-3742 (Mon–Fri 8am–5pm) for an application; a Simplified Chinese form is available. You can also apply online from the EPIC website. You can apply any time of year.
- Step 8EPIC – fill it out and send it: mail or fax it to the address on the form. No documents are needed; EPIC checks your income with Social Security and the NYS Department of Taxation. Remember: you must have, or be able to join, a Medicare Part D drug plan to use EPIC.
- Step 9Need help with forms? Get a free counselor: call New York's HIICAP line at 1-800-701-0501, say which county or borough you live in, and you'll be connected to a free local Medicare counselor.
After you apply
- MSP: within 45 days of applying you'll get a decision letter (Notice of Action). Once approved, you must renew every year; if no renewal letter comes, contact HRA or your county DSS yourself.
- If MSP is denied: you can ask for a fair hearing within 60 days of the decision, using the phone number on your notice or the state hearing line at 1-800-342-3334.
- Extra Help: you'll get a letter with your result and what you'll pay. If you don't have a drug plan, Medicare enrolls you in one automatically, and you can switch to a different plan; with Extra Help you may be able to change plans once a month. Social Security also sends your information to New York to start an MSP application, unless you opt out on the form.
- How long Extra Help lasts: once approved, it lasts the rest of the year even if your income changes, and continues next year unless you get a letter saying otherwise. To ask which level you have, call 1-800-MEDICARE (1-800-633-4227).
- EPIC: if you disagree with an EPIC decision (including a denial), write to EPIC or call the Helpline to ask for a reconsideration; you'll get a written answer.
Tips
- You can get more than one: MSP gives you Extra Help automatically, and people with full Extra Help don't pay the EPIC yearly fee.
- Copy every form before you send it and write down the date you mailed it.
- Applying is free. Don't pay anyone to apply, and don't trust strangers who call asking for your Social Security or Medicare number.
- Every fall, check whether your drug plan still fits you; for free advice call HIICAP at 1-800-701-0501.
